1. Athlete Information

By filling out this form, parents or legal guardians agree to provide accurate information about the athlete, including full name, date of birth, address, contact details, and uniform sizes (shirt, shorts, socks).

2. Parent Behavior Agreement

Parents/legal guardians commit to:

Encourage sportsmanship and support all players, coaches, and staff with respect.
Prioritize players' emotional and physical well-being over the desire to win.
Cooperate with coaches and staff to ensure a positive environment.
Remember the sport is for the children's development and enjoyment.
Teach their child to treat teammates, coaches, opponents, and referees with respect.
Maintain a positive attitude in all interactions with the Academy.
Respect coaches' leadership—no sideline interference.
Avoid conflicts with staff, other parents, or players.
Do not enter the field unless invited by the coach.
Do not criticize referees or game decisions.
Alcohol, illegal drugs, tobacco, and unauthorized medication are strictly prohibited on Academy grounds.

Violations may lead to disciplinary actions, including exclusion from Academy events and activities.

3. Athlete Code of Conduct

The athlete agrees to:

Show respect and good sportsmanship at all times.
Maintain a positive attitude in all Academy activities.
Communicate any concerns respectfully and directly with their coach.

Violations subject to disciplinary action include:

Use of offensive or inappropriate language.
Engaging in arguments or physical confrontations.

Both the athlete and their guardian acknowledge that failure to comply may result in discipline, suspension, or removal from the PR Soccer Academy program.

4. Media Release Consent

During Academy activities, your child may be photographed or filmed. Please refer to the highlighted Media Release Consent section above to make your selection.

"I consent to the reproduction and release of my child's photo or video for PR Soccer Academy's media purposes (e.g., Instagram, Facebook, TikTok, flyers)."

5. Emergency Contact Information

Emergency contact information must be provided, including at least one primary contact with full name, relationship, and phone number.

"The above information is accurate and may be used in the event of an emergency."

6. Liability Waiver and Assumption of Risk

By enrolling in PR Soccer Academy, the legal guardian agrees to the following terms:

a. Assumption of Risk

Acknowledges that soccer involves inherent risks, including injury or death, and assumes full responsibility for participation, even in cases involving negligence.

b. Release and Waiver

Releases PR Soccer Academy, its staff, coaches, volunteers, and affiliates from any and all liability, claims, or damages, including those related to medical assistance or emergency response.

c. Medical Release

Authorizes emergency medical treatment if necessary during participation. Guardian accepts full responsibility for related costs.

d. Acknowledgment

The undersigned has read, understood, and freely accepts the above terms, fully waiving their right to bring legal action against PR Soccer Academy to the maximum extent permitted by law.

7. Payment Agreement and Financial Policies

Payment Platform: SwipeSimple

By completing this registration, the guardian agrees to the following:

a. Agreement to Use SwipeSimple

All payments for services and products will be made via SwipeSimple.
PR Soccer Academy does not have access to cardholder payment information.
A 3.65% transaction fee will apply to each payment.

b. Monthly Payment Schedule

Monthly fees are automatically charged on the 1st of each month.
Reminder emails will be sent weekly and on the final three days before each billing date.
Failed payments incur an automatic $15 late fee.
Accounts with three or more missed payments may face service suspension or cancellation.

c. Additional Services

Extra services (e.g., uniforms, camps, tournaments) are billed separately and not included in the monthly auto-payment.
Payment is required by the specified due date to maintain access to those services.

d. Cancellation and Refund Policy

To cancel, the client must notify the Academy at least 10 days before the next billing date (1st of the month).
Cancellations after this period will not be refunded.
No refunds or credits are provided for service interruptions due to weather, public health mandates, or other uncontrollable circumstances.
One-time purchases are non-refundable after processing.

e. Liability Disclaimer

PR Soccer Academy is not responsible for bank fees, overdraft charges, or any financial penalties due to declined transactions.
This agreement remains in effect until the guardian formally cancels enrollment in accordance with the terms.
